This tool is designed to support nurses, social workers, case managers, and others conducting effective discharge planning and care coordination for adults with disabilities who received care or treatment for COVID-19 illness in an acute care setting, are no longer COVID-19 positive, and require continuation or reconnection to supports and services. While not exhaustive, the resources and considerations proposed in this tool comprise promising practices to be addressed when practicable.

QSEP Training

The Centers for Medicare & Medicaid Services (CMS) wants to help nursing homes prepare for the fall and winter months of the COVID-19 pandemic. Developed to meet the urgent and ongoing needs of nursing home managers and frontline staff, insights gained during the COVID-19 pandemic have been used to create a series of scenario-based trainings to share knowledge around ways to protect, provide care and prevent the spread of COVID-19. These free CMS Targeted COVID-19 Training for Frontline Nursing Home Staff & Management learning intensives are hosted on the CMS Quality, Safety & Education Portal (QSEP). This fall, nursing home staff members can earn certificates of completion while receiving the latest guidance from CMS on COVID-19 preparedness, resident-centered care and infection prevention and control.

CMS Nursing Home Adverse Drug Event Trigger Tool

This tool was developed by CMS for use by nursing home surveyors and is a valuable resource for organizations that are focusing on preventing ADEs.

It was designed to help surveyors identify:

1. The extent to which facilities have identified resident-specific risk factors for adverse drug events

2. The extent to which facilities developed and implemented systems and processes to minimize risks associated with medications that are known to be high-risk and problem-prone

3. When a preventable adverse event has occurred, evaluate if the nursing home identified the issue and responded appropriately to mitigate harm to the individual and prevent recurrence.
