Adapted based on materials from Agency for Healthcare Research and Quality (AHRQ), Project RED (Re-engineered Discharge), and the Coleman Transitions Intervention in support of the Special Innovation Project: Improving Nursing Home Discharges Back to The Community Implementation Guide.

Re-Engineered Discharge (RED) Toolkit

The hospital discharge is a complex process requiring integrated communications among the inpatient care team, primary care team, community services, the patient, and the patient’s caregivers. The RED Toolkit has been expanded to provide complete implementation guidance and adapted to address language barriers, cross-cultural issues, and disparities in health care communication and trust. The toolkit now includes five additional tools that provide step-by-step instructions as a springboard for hospitals to proactively address avoidable readmissions.

Designing and Delivering Whole-Person Transitional Care: The Hospital Guide to Reducing Medicaid Readmissions is intended to help hospital-based readmission reduction teams design and deliver transitional care to address “whole-person needs” (clinical, behavioral, and social). This whole-person approach to transitional care is patient centered, data informed, evidence based, and field tested.

IPRO REDCap Hand Hygiene Project- Nail Care Reminder

This resource is intended to support hand hygiene competency adherence. The 1-page resource summarizes the evidence related to bacteria, resident rights and CDC recommendations to support healthcare staff keeping nails < ¼ inch long. The resource is excellent for staff education that can be shared before or after completing a hand hygiene competency assessment.

The COVID therapeutics action plan provides a template with goals, resources and sample PDSA cycles to assist in the initiation of a plan to optimize the treatment of COVID. The action plan includes a focus on antiviral start time, NIH guideline adherence, monitoring and dosing . A REDCap data collection tool is available to assist with data collection and reporting.
