PRAPARE Tool

The Protocol for Responding to and Assessing Patients’ Assets, Risks, and Experiences (PRAPARE) is a national effort to help health centers and other providers collect the data needed to better understand and act on their patients’ social determinants of health. As providers are increasingly held accountable for reaching population health goals while reducing costs, it is important that they have tools and strategies to identify the upstream socioeconomic drivers of poor outcomes and higher costs.

This Psychotropic Change Package provides a resource with steps toward reducing and managing psychotropic medications in long term care. It includes step-by-step actions with several resources to help the facility plan its psychotropic reduction process, including
an Antipsychotic Action Plan template with SMART (Specific, Measurable, Achievable, Relevant, and Timebound) goals.

Psychotropic Medication Tracking Form and Guide

The “Psychotropic Medication Tracking Form” was developed to assist facilities with a way to track the prescribing of psychotropic medications and any gradual dose reduction efforts. It maintains an active list of all psychotropic medication used in “real time” to include antidepressants, anti-anxiety medications, stimulants, antipsychotics, mood stabilizers and hypnotics. Psychotropic Review Worksheet

Resident-level review worksheet to facilitate providers with ensuring documentation, assessments, diagnoses, and other elements are considered when reviewing a resident record who is on a psychotropic.
This worksheet can be used as part of the facility’s overall Quality Assurance and Performance (QAPI) program, to assist MDS coordinators in completing the CMS Minimum Data Set (MDS) or to prepare for medical provider resident review for gradual dose reduction.
