The Care of Residents with Opioid & Stimulant Use Disorders in Long-Term Care Settings Toolkit

This toolkit outlines six tips to help your LTCF care for your residents with OUD and the co-occurring use of stimulants. Administrators, directors of nursing (DON), medical directors, social workers, nurses, and certified nursing assistants (CNA) can all use these resources. This toolkit will help you comply with state and federal policies and provide evidence-based care to residents with OUD and SUD. Each tip has a list of suggested policies, processes, and educational resources to help your LTCF be better prepared to work with and provide continuity of care to those residents. Finally, in the appendices you will find a table of pharmacotherapy options, process maps for transitions of care, and template forms.

Nursing Home Respiratory Tract Infection Action Plan

The RTI Treatment Action Plan template can assist Nursing Homes in assessing the management of respiratory tract infections in the facility. The template includes examples of root cause analysis, SMART goals, PDSA cycles, resources, and a staff pre and posttest knowledge assessment. The Action Plan is created in Word format to allow the user to modify the content to meet facility specific needs.

New Resource for Nursing Home Educators

IPRO and the Opioid Response Network (ORN) have
developed an education series for use by nursing home
staff educators. This education series will address the
often missed, yet impactful issues of addiction, trauma,
stigma, and chronic pain management.
This pre-packaged, two-month, six-module education
series includes:
• Three live webinars, with Delores Blackwell, PhD
(before, during and after the series) for educators to
review the education materials supplied and receive
support in presenting the content.
Educational content for all staff in a series of six
education sessions designed to last 25-30 minutes
each. Included for each session:
▪ Six prerecorded modules (15 minutes)
▪ Written materials to support discussion led by the
educator (10-15 minutes)

Monthly Falls Tracking Guide & Form

The Monthly Falls Tracking Form was developed to assist facilities track their falls on a monthly basis. It maintains an active list of all falls in real time to assist with both investigation and provide an important piece in the development of a targeted fall prevention program. (See instructions below on how to download the tracking form.)
